Diabetes Connections presents: Club 1921

Let’s get together for an evening of great food, real conversation, and meaningful connections. Whether you're a caregiver for a child with diabetes or an adult with type 1, you’re invited to join us.

We’ll spend the evening exploring one of the newest options in diabetes technology: the twiist™ AID System. Whether you’re actively looking at new pump options or simply want to stay informed about what’s available, this is a chance to learn, ask questions, and hear from others in the community.

As always, this isn’t a formal lecture or a sit-in-a-conference-room kind of night. It’s an opportunity to relax, connect, and leave with practical information you can use, while getting to know others in the local T1D community.

By registering for this event, you agree that we may share your email address with our event sponsor, Sequel Med Tech, makers of the twiist™ insulin pump system. They help make this event possible, and they may reach out with relevant information or offers. We won't share your info with anyone else.

If this event is full, be sure to join the waitlist! We may be able to open up more seats. Those on the waitlist will be the first to hear about any openings and will get early access to register for our next event.

Important note: This is a free event with limited space and a full dinner provided. If you’re unable to attend, we ask that you cancel your registration by 12pm October 30, 2026 so we can offer your seat to someone else.

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT
1.
In consideration for receiving permission to participate in the Diabetes Connections presents Club 1921 event, I hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Stacey Simms Media LLC, their officers, agents, volunteers, or employees (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any of the property belonging to me, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES, or otherwise, while participating in such activity, or while in, on or upon the premises where the activity is
being conducted.

I am fully aware of the unusual risks involved and hazards connected with this activity, I hereby elect to voluntarily
participate in said activity with full knowledge that said activity may be hazardous to me and my property.
I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH that may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in such an activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES OR OTHERWISE.

I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS the RELEASEES from any
loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my
participation in said activity, WHETHER CAUSED BY NEGLIGENCE OF RELEASEES or
otherwise.

It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the
members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am
deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO
SUE the above-named RELEASEES. I hereby further agree that this Waiver of Liability and Hold
Harmless Agreement shall be construed in accordance with the laws of the State of North Carolina
